Performance Review and Self-Assessment Prompt
A self-assessment is where your results become official; this prompt ties achievements to evidence and frames development strategically.

If the review could have been written by someone who was not there, it will not be believed.

The highlighted fields are the places where your own details go. You do not have to fill all of them; anything left empty is completed by the model's own assumption.

03 — How to use
How to use it, step by step
- 01
Keep a running achievement file during the year; reconstructing it in December loses the strongest evidence.
- 02
Choose the development area your manager already raised — a self-invented weakness is unnecessary risk.
- 03
Do not put the promotion request in the form; raise it in a separate conversation.
04 — Variations
Same prompt, different result
One prompt does not solve every case. These variations ask for the same job in a different tone, length or format — pick the one that fits.
Manager review
Same structure written by a manager about a team member: strengths, development area, concrete expectation, support plan. Developmental, not destructive.
360 feedback
Same structure for peer feedback: two strengths with examples, one development area with an example, one suggestion.
05 — Tips
Notes that take the result up a level
- No development area reads as either unread or unrealistic. Include one.
- Result language beats effort language: "cut reporting from 3 days to 4 hours", not "prepared reports".
06 — FAQ
Frequently asked about Career and CV
Is a development area mandatory?
In practice yes. Choosing one your manager already mentioned builds trust; inventing one adds risk.
What if I have no numbers?
Use duration, count, scope or reach. If there is genuinely nothing, leave [ ] and fill it with your manager.
